If you are a new patient to Professor Jones or if your last referral was dated more than 12 months previously, you will need to bring a new referral from your general practitioner or other specialist to the appointment.
All patients are required to fill out the Patient Registration form prior to their appointment. Please download and print the form below and bring to your appointment.
Please note it is important for patients to bring the following things along to the appointment;
– Medications (or a current list)
– Xrays (or CDs of Xrays)
– Copies of previous operation, colonoscopy and pathology reports
